Office Blood Draw - 25.00 + $7.00 flat fee rate for mileage + tax

Clinics - Providers/Clinic will assess a charge if requesting for after hour draw. After hour fee is $50.00 that includes the mileage. Only during business hours will the patient be responsible for $25.00+ $7.00 flat fee rate for mileage + tax.
